    • Sathya Sriram, a health-care strategist and transformation leader, emphasizes the need for safe care in India.

    • Globally, one in ten patients experience harm during hospitalization and four in ten in outpatient care.

    • In India, chronic conditions like cancer, diabetes, heart disease, and mental health disorders demand long-term frequent treatment, creating more points of safety lapses.

    • Patient harm can occur in complex situations like hospital-acquired infections, blood clots, unsafe injection or transfusion practices, and everyday situations like unintentional prescription of inappropriate medicine combinations, delayed diagnoses, or preventable falls.

    • Hospitals are placing higher standards on delivery, but systems designed to protect patients are still vulnerable to breakdowns.

    • A two-way gap exists between overburdened providers and passive uninformed patients hesitant to ask questions.

    • A shared responsibility is needed to empower patients to become active partners in care and health-care systems to welcome and enable their participation.

    • India has taken steps to build an ecosystem for patient safety, including the National Patient Safety Implementation Framework (2018–25), professional networks like the Society of Pharmacovigilance, and the Patient Safety & Access Initiative of India.

    • The WHO Global Patient Safety Action Plan provides a blueprint for integrating safety into all levels of health-care systems, with the establishment of Patient Advisory Councils (PACs) to bring patient voices into hospital decision-making.

    • Governments, hospitals, patients, and civil society must work together to make safe care an everyday reality of health care in India.
