    • The West Bengal Health Department admits to a lapse and plans strict action against anyone found guilty.

    • Five women were administered Ringer’s Lactate (RL) on expired intravenous fluid, leading to critical illness and one death.

    • Three other women were shifted to SSKM Medical College and Hospital for better treatment.

    • West Bengal Chief Secretary Manoj Pant stated authorities have yet to verify whether the lapse was due to human error or technical fault.

    • RL usage and nine other fluids have been banned in West Bengal until investigation is completed.

    • Families of the affected women allege they were made to sign a bond by Medinipur hospital officials.

    • Two public interest litigations were filed at the Calcutta High Court.
